Annual Report
The Heart of the OPS
For 27 years, your Charity at Work
You have your own reason to care

If you are in the OPS workplace, you have the option of donating to Federated Health by payroll deduction. It's the easiest way to give to all sixteen charities or to any three of the charities listed. Your donation can be spread out over whatever number of pay periods you wish. And, your tax credit appears on your T4 slip, so there's no need for a tax receipt. To receive a payroll deduction form, please contact your Ministry Chair or the Campaign Office.

Anyone can donate to Federated Health by cheque or credit card. It is not necessary to be an employee or retiree of the Ontario Public Service. Simply complete the following donation form. A receipt for income tax credit will be mailed to you.

Donations to Federated Health can be designated to all 16 charities, or to whatever combination you wish.

Donate to Celebrate or Remember
Often when someone has died, the family asks that donations be made to a particular health charity in lieu of flowers. It's also common for people to ask for a donation to charity in lieu of birthday or wedding presents. You can make that type of donation through Federated Health.

Simply fill out the "Donate to Celebrate or Remember" section on the downloadable donation form above.

If you wish, the Federated Health Charities office will send a card to the family of the deceased, or to the individual you're honouring, informing them that you have made a donation in honour of their loved one. The family, or the individual, will be told which charity (or group of charities) was designated to receive the donation.

These donations can be made at any time throughout the year.
